Job Description
Directs, supervises, evaluates, and provide nutritional services on an intermittent basis to patients in their homes in accordance with the physician's orders
Responsibilities
Assesses and evaluate the nutritional status of patients
Assists the physician and nursing personnel to determine the dietary needs of the patient
Prepares and calculates therapeutic diets to meet specific requirements of the patient and confers with the physician to establish dietary treatment
Assists the patient and family to understand and follow dietary modifications ordered by the physician. Instructs on dietary restrictions and drug/nutrient interactions as appropriate
Observes and reports to the physician and nurse supervisor the patient's response to dietary treatment and any related changes in the patient's condition and revises nutritional plan as indicated by patient's condition
Participates in care coordination activities, and acts as a resource to other health care staff, as requested
Participates in Quality Improvement activities as assigned and communicates information that enables collection and analysis of data to identify opportunities for improvement
Participates in the development of periodic revision of the physician's plan of treatment keeping him/her informed of the patient's status and obtaining additional orders if necessary
Communicates with other team members and, when appropriate, instructs them in nutritional, dietary teaching that they may use while working with the patient
Maintains and submits written clinical and agency records as deemed necessary
Participates with staff, patient, and physician in discharge activities and completes a Dietary Discharge Summary
Assumes responsibility for self-development by continually striving to improve his/her health care knowledge through educational programs, attendance at workshops and conferences, active participation in professional and related organization meetings and individual research and reading
Confirms, on a weekly basis, the scheduling of visits with the Director of Nursing to coordinate Agency visits
Performs additional duties assigned in response to patient and/or Agency needs
Qualifications
Must have at least a bachelor’s degree from a course of study accredited by the Accreditation Council for Education in Nutrition and Dietetics (ACEND)
Licensed in the state of Pennsylvania
Must have at least one year of experience
Must have a criminal background check
Must have a current CPR certification. Online certification is not accepted
