    Job Context:The Licensed Clinical Social Worker (LCSW) is under the direct supervision of the Chief Integrated Health Officer and performs all functions of their duties and responsibilities within the primary care environment as overseen and directed by the Chief Medical Officer. The LCSW is responsible for patient care and a variety of administrative tasks related to patient care delivery. The LCSW, as part of the care team, identifies, triages, and manages patients with behavioral health challenges within the primary care setting. In addition, the LCSW will provide and develop specific behavioral change care plans and interventions for patients while following behavioral health protocols and best practices to improve patient behavioral health outcomes. The LCSW supports both the primary care providers and the patient by providing high quality clinical mental health consultation, interventions, and referrals to and for patients as requested by the various healthcare providers. Responsibilities & Duties:

    Assist primary health care providers in recognizing behavioral health disorders and psychosocial problems.

    Assess the behavioral health status of patients referred by primary care providers through brief (10-20 minutes) solution focused behavioral health interventions with active development of behavioral change planning for patients.

    Work with primary care team to identify and manage patients with chronic emotional and/or behavioral health problems efficiently and effectively.

    Work with primary care provider to refer cases to mental health specialists as appropriate.

    Assist in the detection of “at risk” patients and development of plans and referral resources to prevent further psychological or physical deterioration.

    Assist in preventing relapse or morbidity in conditions that tend to recur over time through appropriate case management.

    Evaluate patient care plans for behavioral health interventions with primary care team.

    Teach patients, families, and caregivers prevention, and coping techniques.

    Attend and participate in meetings and Quality Improvement activities as required.

    Promote education and understanding of behavioral health issues to care team members.

    Promote smooth transition between GSCC care teams and specialized mental/behavioral health organizations to optimize patient health outcomes.

    Provide consistent and expeditious feedback to referring providers on patient referral status.

    Provide on-site availability for crisis triage and intervention within scope of practice.

    Document behavioral health interventions thoroughly and track patient referrals to specialty behavioral health providers and completes documentation within 48-72 hours.

    Assures that patients receive services in an appropriate, efficient, expeditious and professional manner.

    Recognizes the need for development and modification of services and proposals/recommends changes to supervisor.

    Responsible for self-care to ensure optimal, ethical, and safe health delivery service to patients.

    Work effectively on a team and communicate respectfully with patients and all staff based on shared goals and mutual respect.

    This includes demonstrating adaptability to continuous efforts to improve patient care and willingness to be an active care team member engaged in ongoing process and quality improvement projects within the framework of the Patient-Centered Medical Home.

Occasionally lift up to 30 pounds.Knowledge & Experience:Required Qualifications:

    Master’s degree in social work from an accredited school of social work.

    Licensed by the Oklahoma State Board of Social Work.

    Ability to communicate well both in writing and orally.

    Knowledge of and the ability to provide integrated service delivery to patients involving social and emotional problems utilizing motivational interviewing.

    Knowledge of the cultural, social, and economic backgrounds of therapeutic and crisis intervention services involving social and emotional problems stemming from medical conditions or diagnosis.

    Ability to provide written or oral reports and evaluations concerning program activities, accomplishments, goals, and objectives.

    Knowledge of existing social and behavioral health resources available for patient care.

    Capable of making independent professional judgments and recommendations for medical, social and psychosocial intervention.

    Able to manage multiple tasks to completion in a fast-paced environment, while maintaining a high degree of professionalism and exceptional customer service with diverse constituencies.

    Preferred Qualifications:

    Licensed Clinical Social Worker

    Two (2) years of supervised social work experience in a health care setting, working directly with individuals.

    Prior work with diverse and complex patient populations.

    Familiarity or work experience in integrated behavioral health models.

    Full COVID-19 vaccination required
