PURPOSE OF YOUR POSITION
Responsible for the following:
- Delivery of treatment services to children ranging in ages from 0-21 years
 
- Evaluation services
 
- Job-related department responsibilities
 
       
 
POSITION FUNCTIONS AND RESPONSIBILITIES
- Provide treatment for 10 – 12 patients per day
 
- Supervise student interns as directed by Manager
 
- Conduct initial Physical Therapy evaluations and re-evaluations
 
- Develops and reviews patient care plans in conjunction with Physiatrist
 
- Participates in clinical quality assurance initiatives
 
- Provides patient education through verbal explanation, visual demonstration, or written documentation
 
- Complete reporting activities as required by regulatory and departmental standards including but not limited to:
 
- Daily notation of therapy
 
- All progress reports with updates
 
- Rehabilitation rounds/staff conferences
 
- Other activities that may be assigned form time to time, i.e. splinting
 
- Comply with established CRC, Inc. policies and procedures re: quality assurance, safety, environmental and infection control, confidentiality of patient information, customer service, attendance, punctuality, dress code, professional conduct, annual health assessments etc.
 
- Participate in all department in-services and staff development activities
 
Required Skills
 
EDUCATION REQUIREMENTS:
- Bachelor’s Degree and/or Master’s/or DPT (Doctorate in PT)
 
- NYS License as a registered Physical Therapist (PT) or in process with permit.
 
- Must maintain license in good standing.
