    Job Description

     Under the direction of the Program Manager, the Case Manager I provides case management, community-based resource and referrals and advocacy efforts for youth experiencing homelessness or at-risk of homelessness in the Sacramento area. This position is responsible for intake and assessment, curriculum/group development, and assisting youth in implementing a case plan. The Case Manager I assists youth in achieving their personal goals through positive youth development within a harm reduction framework.

    RESPONSIBILITIES:

    • Works directly with youth experiencing homelessness and their families.
    • Maintains an assigned caseload, in addition to daily supportive contact with youth at the Wind Center
    • Through rapport building, provides strength based case management to youth. Case management involves screening, intake, assessment, individual’s service plan development, monitoring, linkage to appropriate community resources, follow-up, advocacy and tracking youth outcomes.
    • Maintains precise and accurate documentation of case management services, including client files and entries into the program database.
    • Adhere to the policies and procedures outlined in the Wind Center Program Manual.
    • Assists youth in finding shelter and completing necessary requirements to obtain secure and safe housing.
    • Assists youth in obtaining other supportive documents such as identification, social security cards, birth certificates, educational information, etc.
    • Maintains up to date information regarding availability and status of community programs and resources that are necessary and relevant to youth.
    • Assists youth in accessing and maintaining entitlements and benefits.
    • Assists clients in achieving goals relating to education, employment, housing, and other youth identified goals.
    • Assists in daily activities at the Wind Center to maintain its functionality, relevancy of services, and positive group development.
    • Focuses on meeting the specific needs of each youth as an individual.
    • Participates in supervision.
    • Facilitates groups and workshops for youth as needed.
    • Transports youth as needed.
    • Performs other related duties as required.
  • Qualifications

    Qualifications

    • PREFERRED: Bachelor’s degree in related field from accredited university or college AND 2 years’ experience providing direct service.
    • ACCEPTED: 4 years’ experience providing direct service.
    • Experience working within diverse communities.
    • Working knowledge of community resources as they relate to homelessness and youth.
    • HMIS experience preferred.
    • No record or disclosure of criminal conviction that indicates a tendency toward theft, violence, dishonesty, deceit, drug manufacture or sale, or predatory behavior.
    • Must provide proof of identification and eligibility to work in the US.
    • Must pass a drug and background screening.
    • Proficiency with Microsoft Office Suite.
    • Must be at least 21 years of age.
    • Experience working with homeless and/or at-risk youth; ability to establish and maintain trusting relationships with youth.
    • Direct experience providing clinical services, case management or crisis services.
    • Ability to remain calm and focused in volatile situations. Demonstrated experience working in crisis intervention.
    • Ability to develop and maintain professional behavior/appropriate boundaries with youth and staff members.
    • Ability to work independently and as a member of a team.
    • Acceptance of a variety of lifestyles, behaviors, and cultural and spiritual practices.
    • Understanding of harm reduction frameworks and how they apply to direct service.

    Additional Information

    POSITION INFORMATION:

    • Job Type: Full-Time
    • Required work schedule: Monday-Friday (6:30am-3:00pm)
    • Compensation (DOE): $15.00 - $18.00 per hour
    • Work is primarily performed in Drop-In Center and community locations.
    • Work environment usually hectic (fast paced) with frequent short deadlines and a regular number of critical or unusual situations.
    • Some physical work, including lifting, bending, climbing stairs, carrying up to 35 pounds.
    • Must have reliable transportation and valid driver’s license and be willing to travel to various work locations.
    • May require work outside of normal work hours, including evenings or weekends.
