    $16.00+ AN HOUR (WITH EXPERIENCE) Do you have a passion for community service? Our Resource Specialist will work closely with our patients to help them find the appropriate resources for their family needs. We offer competitive wages based on experience and up to 3 weeks of PTO in the first year!     EXAMPLES OF DUTIES: (This list may not be all inclusive.)

    1. Performs assessment of patient and family to identify basic life, educational, transportation, housing and safety needs.
    2. Engages and coordinates resources available at Grace Health and in the community to meet identified needs such as prescriptions, food, or transportation vouchers.
    3. Assists with education, and referral for SFD, financial assistance programs, and medical insurance programs.
    4. Educates staff regarding community resources.
    5. Attends relevant community meetings and represents the interest of Grace Health and its patients.
    6. Attends relevant Grace Health patient care / family care conferences.
    7. Maintains documentation related to assessment, referral to resources, and coordination with other human resource agencies in the patient chart.
    8. Maintains appropriate tracking logs and provides reports as needed.
    9. Manages appropriate alerts in chart.
    10. Communicates Grace Health processes to patients.
    11. Participates in professional development activities.
    12. Facilitates referrals to internal and external resources.
    13. Establishes collaborative working relationships with community partners.
    14. Meet with patients to discuss Needs Affecting Your Health form.

    REQUIREMENTS

    • COVID-19 AND FLU VACCINE
    • ASSOCIATE DEGREE OR OTHER HIGHER EDUCATION PREFERRED
    • ONE YEAR OF COMMUNITY HEALTH WORK
