    BASIC JOB PURPOSE: Increase health insurance coverage and access to quality primary health, dental, and behavioral care for internal patients and members of the local community

    SUPERVISION RECEIVED: Reports to the Manager of Patient Accounts

    SUPERVISION EXERCISED: None

    TYPICAL PHYSICAL DEMANDS:  Requires sitting for long periods of time. Some bending and stretching required. Use of telephone required. Manual dexterity required for use of calculator and computer keyboard.  At times, this position may include periods of extended physical activity, such as walking/standing at community events and/or moving and distributing educational materials. 

    TYPICAL WORKING CONDITIONS:  Working in a normal office environment. Occasionally, time might be spent in-field providing outreach and enrollment activities, including travel by car within Grace Health's service area.  May require evening and/or weekend work. Work may be stressful at times.

    EXAMPLES OF DUTIES:  (This list may not be all inclusive.)

    1. Assists patients with application process for schedule of discounts program.

    2. Provide enrollment assistance (including but not limited to completing coverage applications, gathering required documentation and troubleshooting the enrollment process) for uninsured children and adults to access subsidized, low-cost and free health insurance programs through the health insurance marketplace, Medicaid and the Children's Health Insurance Program (CHIP).

    3. Provide structured patient education on health coverage, engage in follow-up conversations and offer renewal assistance for enrolled individuals.

    4. Attend and successfully complete all required training programs; participate in ongoing conference calls, webinars, and other professional development opportunities.

    5. Accurately provide required reporting to track goal achievement and client satisfaction.

    6. Assist in the development and implementation of organizational outreach and enrollment initiatives.

    7. Distribute outreach materials to patients, community members, partner organizations and businesses to build coverage option awareness. 

    8. Attend community events in order to build awareness of coverage options, spur enrollment, build referral linkages and promote the mission and services of the organization.

    9. Maintains strictest confidentiality.

    PERFORMANCE REQUIREMENTS

    Knowledge, Skills and Abilities:

    • Knowledge of the health and human services infrastructure, health insurance programs and public coverage options.
    • Strong computer skills with proficiencies in Outlook, Word, PowerPoint, Excel, internet-based applications and the Microsoft operating system.
    • Ability to effectively develop and nurture respectful relationships with a diverse group of individuals.
    • Ability to speak effectively before small and large groups and actively engage the general public in outreach situations.
    • Ability to work independently and coordinate multiple tasks.
    • Ability to read and interpret documents along with write routine reports and correspondence.
    • Ability to communicate effectively, both verbally and in writing, with patients and staff.

    _Education:  _High school diploma or GED, Associate or Bachelor degree in public health, health education, communications or related program preferred

    Experience: One year of experience in a health care organization preferred or equivalent combination of education and experience

    Certificate/License: Valid driver's license
