Custom Care Pharmacy is seeking a motivated Graduate Pharmacist Intern to join our closed-door long-term care pharmacy in Milton, Pennsylvania. This is a delivery-only LTC pharmacy , not a traditional retail setting, so you can focus on workflow, accuracy, patient care, and meaningful clinical work without front-end customer traffic.

Position Overview
As a Graduate Pharmacist Intern, you will work closely with pharmacists, technicians, and leadership to support daily pharmacy operations in a high-volume LTC setting. You will gain experience in medication order review, packaging workflows, facility communication, transitions of care, and quality-focused pharmacy operations.
Key Responsibilities
- Review medication orders for appropriateness, accuracy, and safety
- Assist with prescription processing, order entry, and workflow support
- Support LTC operations including cycle fills, new admits, medication changes, and STAT orders
- Help ensure accurate dispensing, packaging, and delivery preparation
- Communicate with nurses, facilities, prescribers, caregivers, and pharmacy team members
- Participate in medication reconciliation and transitions of care
- Maintain accurate patient records and documentation
- Provide drug information and help resolve medication-related issues
- Support quality assurance, compliance, and process improvement initiatives
- Contribute to a fast-moving team environment focused on accuracy, service, and patient care

Qualifications
- Pharm.D. graduate or upcoming Pharm.D. graduate eligible for graduate intern status
- Pennsylvania intern license in good standing, or ability to obtain required credentials
- Working toward Pennsylvania pharmacist licensure
- Strong attention to detail and ability to work efficiently in a fast-paced environment
- Excellent communication, organization, and time-management skills
- Long-term care, institutional, or retail pharmacy experience preferred, but not required
- Positive attitude and willingness to learn in a high-performance team setting
