About the Role:

Star Light Home Health is looking for a passionate and dedicated Physical Therapist (PT) to join our growing team in Grand Junction, CO. Responsibilities:

Conduct comprehensive in-home physical therapy evaluations and assessments for Medicare-eligible patients

Develop and implement individualized treatment plans to restore function, improve mobility, and reduce pain

Provide therapeutic exercises, manual therapy, and functional training in the home setting

Monitor and document patient progress, adjusting care plans as needed to meet goals

Collaborate with physicians, nurses, and other healthcare team members to coordinate patient care

Educate patients and caregivers on home exercise programs and fall prevention strategies

Ensure accurate and timely documentation in compliance with Medicare and agency standards

Requirements:

Active Physical Therapist (PT) license in the state of Colorado

Graduate of an accredited Physical Therapy program (DPT or MPT)

Experience in home health, geriatrics, or Medicare-certified settings preferred

Strong knowledge of Medicare guidelines and home health documentation requirements

Excellent communication, time management, and organizational skills

Valid driver's license and reliable transportation for home visits in the Grand Junction area

Compassionate, self-motivated, and able to work independently in a field-based role
