    Position Summary:

    The Clinical Pharmacist will work in a multidisciplinary environment, collaborating with our network providers, health systems, and contract payers to assist in building a superior care pathway to improve the overall health of patients. The Clinical Pharmacist will be involved in the development of disease management programs, and will implement comprehensive medication management services supporting our General Physician, PC practices. The Clinical Pharmacist will be responsible for working with physicians, pharmacists, nurse practitioners, physician assistants, and other healthcare professionals as part of a multidisciplinary care team in an effort to improve patient care, lower healthcare costs and assist with the implementation of payer-based quality improvement initiatives.

    Duties and Responsibilities ( including but not limited to )

    • Provide sound pharmacotherapeutic recommendations to providers based on current practice guidelines
    • Work with the Clinical Pharmacy Manager to evaluate opportunities to integrate clinical pharmacy support within GPPC’s primary care practices
    • Provide comprehensive medication management services to patients via in-person consultation or via telehealth
    • Ensure appropriate and cost-effective drug therapy use, areas of focus may include but not limited to evaluating and addressing drug-drug interactions, duplicate therapy, low and/or high doses of medications, opportunities to reduce medication cost
    • Serve as a drug information and medication safety resource expert for the network and clinical team
    • Assist with the provision of Medication Therapy Management (MTM) services to all eligible patients attributed to the General Physician, PC
    • Participate in the education of staff, students, interns, residents, and other healthcare providers; acts as a preceptor and mentor where applicable
    • Provide assistance with quality gap closure to support General Physician, PC’s primary care practices in contract performance initiatives

    Qualifications:

    • Must possess a Doctor of Pharmacy (Pharm.D.) from an ACPE accredited School of Pharmacy
    • Must have completed a PGY-1 residency in an applicable area of practice, ambulatory care experience preferred
    • Current, active license to practice pharmacy in New York State, required
    • Minimum one (1) year professional experience in a clinical setting with at least 50% of time spent involved in direct patient care
    • Board of Pharmacy Specialties (BPS) certification required to be obtained within 24 months of date of hire.
    • Diabetic Educator certification (preferred)

    ** Knowledge and Skills:**

    • Ability to analyze/interpret and utilize results of various laboratory and microbiology tests and apply them to patient care in a clinical practice setting.
    • Ability to effectively utilize clinical practice guidelines and evidence-based medicine in everyday practice
    • Excellent interpersonal skills and the ability to convey educational material in an understandable manner to patients and their families.
    • Ability to operate ancillary patient care information systems in conjunction with the operation of patient testing equipment
    • Ability to communicate effectively both written and oral with Physicians, Physicians Assistants, Nurse Practitioners, Nurses, and other allied health professionals.
    • Must be able to work independently to meet job requirements
    • Must have the ability to travel within Erie County and the surrounding counties as required

    Pay range: $100,000. - $130,000.

    The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within job’s compensation range, and will be determined by considering factors including, but not limited to market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
