    JOB TITLE: Clinical Liaison

    HOURS: Friday, Saturday, and Sunday: 8:00 AM to 8:00 PM

    COVERAGE AREA: East Pasco County

    POSITION SUMMARY:

    Reporting to the Manager of Patient Access, the Clinical Liaison facilitates referrals to hospice by fathering and interpreting medical and financial data, meeting with patients and families, and working with case managers to ensure optimal transition to hospice care.

    EDUCATION AND QUALIFICATIONS:

    Active RN license

    Graduate from an accredited school of nursing, BSN preferred.

    A minimum of two (2) years clinical experience working in and acute care setting; one year of home care and/or hospice experience preferred.

    Must maintain current Florida driver’s license

    Must maintain valid automobile insurance coverage

    ESSENTIAL JOB RESPONSIBILITIES:

    Assists in the development and implementation of agency marketing and referrals.

    Makes initial contact and physical, social and psychological and financial assessment of patients to determine appropriateness of hospice services.

    Successfully builds relationships with case managers, social workers, physicians etc.

    Works with referral partners to provide appropriate alternatives for patients who do not meet hospice criteria including patients with no funding source.

    Assists with patient’s insurance to determine hospice coverage.

    Self identifies options for payment, which are not necessarily identified by patient.

    Provide intradepartmental and interdepartmental flow of information, according to existing policies and procedures.

    Maintain a working familiarity with admission criteria, and any other pertinent Hospice issues. Interprets hospice policies and acts as resource for case managers, physicians, hospital and community for effective public relations and marketing of services.

    Works cooperatively with other Patient Access Team members to provide information about GHS services and other appropriate community resources to address the verbalized need for assistance.

    Consults and collaborates with other professionals to promote quality patient care.

    Completes all requires documentation on correct hospice forms with accuracy, timeliness, legibility and professional terminology.

    Initiates new patient medications, equipment, supplies and transportation upon admission as needed.

    Demonstrates clinical skills regarding case management and application of GHS policies and procedures such as: intravenous skills, pain management, symptom management, and patient/family relationships.

    Adheres to GHS clinical and general policies and procedures.

    Forwards all completed admission paperwork to Patient Access Specialist within 24 hours of admission.

    Handles patient problems as related to unmet expectations for hospice.

    Effectively communicates with physicians who are non-supporters of organization’s hospice services.

    Conducts individual and group presentations on products and services in an informative, accurate, and professional manner. Responds to questions about products and services.

    Maintains regular contact with discharge planners, social workers, hospitals, nursing homes, assisted living facilities and other referral sources to facilitate the referral process.

    Acts as liaison between physicians, skilled nursing facilities, hospitals and rehabilitation providers communicating service enhancements, new products, new programs and service delivery.

    Facilitates communication between Business Development and Hospital, Skilled Nursing Facility representatives, and other referral sources. Relates opportunities for recovery activities.

    Keeps agency informed of facility changes regarding bed capacity, staff changes, new facility programs and/or procedures.

    Acts in a professional and responsible manner within and outside the company; contributes a positive image; utilizes positive interpersonal skills.

    Adjusts to shifting priorities, ambiguity and rapid change; demonstrates flexibility, correlating with the needs of the patient/family/facility.

    Effectively communicates with patient, family, supervisor and physicians in facilitating a smooth admission.

    Initiates the patient’s plan of care and educates patients and families on the services offered by GHS.

    Provides ongoing assessments of physical, psychological, social and spiritual needs of the patient/family unit and initiates appropriate intervention; documentation is accurate and returned to Patient Access Team within 24 hours.
