    JOB TITLE: Clinical Social Worker

    HOURS: Monday - Friday, 8:00 AM to 5:00 PM (On-Call Rotation Required)

    COVERAGE AREA: West Pasco County

    POSITION SUMMARY:

    Reporting to the Director of Clinical Social Work Services, and clinically supervised by a License Clinical Social Worker, the Clinical Social Worker provides comprehensive psychosocial and community resource services for patients and caregivers in a healthcare environment.

    EDUCATION AND QUALIFICATIONS:

    Must possess a Master’s degree in Social Work

    LCSW preferred, or willing to train license under clinical supervision a Gulfside Healthcare Services Licensed Clinical Social Worker

    Must possess a minimum of three years of clinical Social Work experience in a healthcare organization, facility, hospital or community health program.

    Proven success in psychosocial assessments, crisis intervention, family dynamics, mental health issues, clinical counseling and resource management

    Extensive knowledge and professional contacts with local, community, state and federal resources for patients and caregivers through the course of care

    Admissions and discharge planning experience in healthcare settings

    Ability to form relationships and build contacts with hospitals, nursing homes, assisted living facilities, community health clinics and other providers for serving patients

    Member of the National Association of Social Workers, conducting the NASW code of conduct at all times

    Adherence to the practice of confidentiality regarding patients, families, staff and the entire organization.

    Ability to work with and serve as a mature representative of Gulfside Healthcare Services

    Ability to work flexible hours including evenings and/or weekends as needed or requested.

    Detail oriented, able to multi-task and remain flexible with assignments.

    Must demonstrate skill in conflict resolution.

    Must be able to uphold attendance and performance standards.

    Must be able to recognize, support and adhere to all GHS policies and procedures.

    Must have reliable transportation to perform tasks and responsibilities in a timely and appropriate fashion. Must be able to provide proof of automobile insurance and have a valid Florida driver’s license, a copy of which will be placed in their employee file

    ESSENTIAL JOB RESPONSIBILITIES:

    Works within the clinical team, reporting to the Clinical Care Manager, and also supervised by a Licensed Clinical Social Worker fore case management and clinical intervention.

    Must report for work at scheduled time, ready to perform work functions and remain on task while completing entire scheduled shift.

    Represents Gulfside Healthcare Services in a professional and responsible manner.

    Practices social work/counseling in a manner that is a model for professional and agency ethics, values and integrity and that complies with the letter and spirit of legal aspects.

    Adheres to the practice of confidentiality regarding patients, families, staff and GHS.

    Functions as an integral part of an interdisciplinary team in order to enhance the mission, policy and philosophy of GHS.

    Complies with all applicable laws and regulations with respect to collaborative agreements.

    Is sensitive and compassionate with a patient-centered attitude towards accepting death as a part of life and enhancing the quality of life for patients, assigned to his/her care.

    Participates in weekend duty schedule of social workers.

    Provides coverage for other social work staff as needed, including weekend rotation coverage.

    As part of the clinical interdisciplinary team, performs patient psychosocial intake activities and disseminate of information regarding GHS as needed.

    When appropriate, assists in identifying and works with high-risk and/or suicidal patients/families and with patients/families experiencing other kinds of non-medical crisis.

    Participates in on-going psychosocial, spiritual and bereavement assessment of the patient/family in cooperation with interdisciplinary team.

    Assists with the development and implementation of a patient/family personalized care plan, which may include counseling, support information and/or referral to GHS and/or other community resources.

    Provides case management services relating to psychosocial needs for patient/family.

    Functions with proficiency using electronic health record software.

    Completes clinical documentation, psychosocial assessments, clinical notes and secondary reports in a timely manner, including Bereavement Assessment and cards.

    Provides input to Volunteer Coordinator regarding patient/family needs to promote volunteer assignment and match.

    Provides individual support services to the bereaved as requested.

    Demonstrates continued professional growth and development through participations in educational and in-service training programs for professional staff, providing a record of programs attended to Human Resources. Responsible to review current health care literature relating to duties.

    Attends Interdisciplinary Group, case conferences and staff meetings as requested.

    Maintains and meets requirements of LCSW Board for CEU, as well as GHS requirements.

    Assists in Volunteer training process as requested.

    Individually responsible to conduct himself/herself in a safe manner during performance of all job related duties. Also responsible to bring unsafe conditions to the attention of the person within the organization responsible for safety issues.

    Assumes responsibilities for project/program development and implementation as assigned.

    All other duties/tasks as deemed appropriate for the positions of social worker as requested/delegated or assigned by immediate supervisor or Executive Director.

    Functions as a professional and cooperative GHS team leader.

    Addresses and supports all GHS policies and procedures.

    Responsible to provide continuity of care to patient/family by providing Leave Request regarding anticipated absence.

    Maintains a rolling monthly minimum average of twenty (20) patient, client and/or caregiver visits per week.
