    JOB TITLE: Intake Coordinator - LPN

    DEPARTMENT: Admissions

    REPORTS TO: Nurse Care Manager - Admissions

    POSITION SUMMARY:

    Reporting to the Nurse Care Manager – Admissions, the Intake Coordinator - LPN is committed to providing the highest quality of care as identified in the GHPPC Mission. Responsibilities of this position are to ensure a seamless coordination of patient care from initial referral contact through the admission process. They are identified as the contact person with physicians, case managers, community agencies, patients/families and serve as an initial educational source for referrals.

    EDUCATION AND QUALIFICATIONS:

    Currently licensed in the State of Florida as a Licensed Practical Nurse

    Minimum of one (1) year of nursing experience.

    ESSENTIAL JOB RESPONSIBILITIES:

    Acts in a professional and responsible manner within and outside the company; contributes a positive image.

    Adjusts to shifting priorities, ambiguity and rapid change; demonstrates flexibility

    Receives all incoming referrals and requests supportive documentation to facilitate the admission.

    Is responsible to initiate and update patient referrals by documenting patient information in AllScripts.

    Contacts physicians to request certification for Hospice Care

    Maintains close contact with Admission Nurses and Hospital Liaison’s, informing of all new referrals and follows the progress of existing referrals.

    Arranges for DME deliveries as necessary.

    Keeps record of Admission and Referral Logs and update them on a daily basis.

    Develops Monthly Reports derived from data collected throughout the month in relation to the Admission/Intake Department.

    Maintains a professional relationship and serves as a liaison with other disciplines.

    Attends to patient and family complaints/concerns and works diligently to address these issues in an effective, timely fashion.

    Creates and promotes initiatives which will develop and strengthen working relationships with care facility partners. Maintains a pro-active approach to managing potential issues. Works to enhance communication and takes initiative to ensure problem resolution is performed in a timely and effective manner. Uses persuasiveness and good judgment to formulate win-win outcomes when possible.

    Educates the community regarding Hospice care, philosophy and concept through a case by case consultation with individuals in the community, educational in-services or seminars when requested.

    Possess the ability to multi-task and exhibits adaptability and flexibility in maintaining compliance in correlation with GHPPC standards.

    Ensures all documentation is accurate and is compliant with GHPPC standards, Florida State and Federal Regulations.

    Ensures professional standards of Hospice nursing practice are maintained.

    Serves as a professional resource with assisting in the mentoring of new staff when requested.

    Responsible to ensure all Florida State CE requirements are maintained for valid licensure.
