    JOB TITLE: Admission RN

    DEPARTMENT: Admissions

    REPORTS TO: Nurse Care Manager - Admissions

    POSITION HOURS: (4) 10 hour shifts

    12 pm to 11 pm

    POSITION SUMMARY:

    Reporting to the Nurse Care Manager - Admissions, the Admission RN is responsible to perform timely, individualized, accurate assessments for patients referred to the organization for Hospice services as they relate to the Local Coverage Determinations. The RN will represent GHPPC in a professional manner and is committed to delivering the highest quality of care possible as identified in the GHPPC Mission.

    EDUCATION AND QUALIFICATIONS:

    Currently licensed in the State of Florida as a Registered Nurse, with a minimum of one (1) year of Hospice related nursing experience.

    ESSENTIAL JOB RESPONSIBILITIES:

    Acts in a professional and responsible manner within and outside the company; contributes a positive image; utilizes positive interpersonal skills.

    Adjusts to shifting priorities, ambiguity and rapid change; demonstrates flexibility, correlating with the needs of the patient/family/facility.

    Demonstrates leadership and utilizes critical thinking skills in assessing prospective admissions. Evaluates each case on an individual basis and maintains compliant with admission criteria as identified by the LCD’s.

    Effectively communicates with patient, family, supervisor and physicians in facilitating a smooth admission.

    Initiates the patient’s plan of care and educates patients and families on the services offered by GHPPC.

    Provides ongoing assessments of physical, psychological, social and spiritual needs of the patient/family unit and initiates appropriate intervention; documentation is accurate and returned to Intake Coordinators within 24 hours.

    Anticipates future needs of the patients and is pro-active in ordering appropriate DME and requesting nursing/CNA/Volunteer Services.

    May serve as a Hospital Liaison when requested

    Maintains a professional relationship and serves as a liaison with other disciplines/facilities.

    Attends to patient and families complaints/concerns and works diligently to promote timely effective resolution.

    Educates the community regarding Hospice care, philosophy and concept through a case by case consultation with individuals in the community, educational in-services or seminars when requested.

    Possess the ability to multi-task and exhibits adaptability and flexibility in maintaining compliance in correlation with GHPPC standards.

    Ensures all patient care and documentation of care is compliant with GHPPC standards, Florida State and Federal Regulations.

    Ensures professional standards of Hospice nursing practice are maintained.

    Serves as a professional resource to the community.

    Responsible to ensure all Florida State CE requirements are maintained for valid licensure.

    Maintains up to date skill sets as required to care for patients.

    Possesses ability to multi-task; exhibits adaptability and flexibility in meeting Gulfside Hospice & Pasco Palliative Care objectives.

    Ensures compliance with all patient care, meets documentation standards and regulations.

    Assists in maintaining federal and state regulatory compliance. Adheres to GHPPC policy and procedures.

    Proficient in Electronic Medical Records.
