Harbors Home Health & Hospice, the premier home care agency committed to providing Grays Harbor County with a variety of in-home healthcare services, is currently seeking a Social Worker to join our team! . Located in and around scenic Grays Harbor and Pacific County. Individuals will be responsible for working with a team of health care providers in the coordination of skilled care in a home settings.

Assumes responsibility for providing counseling and clinical support to individuals and families requiring emotional and physical assistance. Assesses individuals, develops program content, and selects and implements treatment programs as appropriate. Recommends community resources as necessary. Maintains the highest levels of confidentiality and upholds a high standard of professional ethics.

Provides counseling and clinical support to individuals and families with problems that may arise while receiving home health or hospice services.

Assesses individuals to determine overall social and emotional capabilities.

Develops a plan of care for patients and families.

Provides advocacy for marginalized individuals within the Agency and refers them to the appropriate community resources as needed.

Requires: License in social work or clinical psychology, or has a baccalaureate degree in social work or psychology and working under an MSW (our agency currently employs an MSW) WA State Medical License Current Drivers license with vehicle insurance knowledge of community resources strong interpersonal and public relations abilities
