Harbors Home Health & Hospice is looking to ad a full time PTA to its valuable therapy team. This individual is responsible for working closely with our Physical Therapists to provide excellence in patient care. This position gives the clinician the opportunity for both autonomy, one on one patient care, and working with a great team! Giving you the chance to spend a few extra valuable minutes for that special patient. Harbors' offers flexibility for a career and a personal life. Duties may include: Assists in developing and carrying out therapy care plans Teams up with a licensed Physical Therapist to provide continuity of care Patient and family education Documents service provided and communicates changes and concerns with team mates
