    Position Overview:

    A respected nonprofit agency providing home and community-based services is seeking a licensed clinical social worker (LMSW, LCSW, or equivalent) to support its HCBS program through clinical oversight, documentation training, and provider guidance. This role bridges clinical expertise with operational support by ensuring high-quality service plans, accurate documentation, and smooth insurance authorization processes.

    The right candidate will have clinical experience in HCBS, an eye for compliance, and the communication skills needed to train, support, and guide providers. This is a critical position for maintaining regulatory standards and improving service delivery outcomes.

     

    The Ideal Candidate:

    • Licensed LMSW, LCSW, or equivalent with HCBS-related clinical experience

    • Deep understanding of Medicaid HCBS documentation requirements

    • Experienced in writing detailed service plans and progress notes

    • Familiar with insurance approval processes, prior authorizations, and supporting documentation

    • Comfortable training and supporting providers in real time

    • Organized, detail-oriented, and proactive in enforcing documentation standards

    • Strong communicator—can explain clinical concepts clearly to non-clinical staff

     

    Key Responsibilities:

    • Draft and review HCBS service plans and clinical documentation for accuracy and compliance

    • Train providers on documentation standards, person-centered planning, and progress note writing

    • Guide providers through initial and ongoing service plan implementation

    • Oversee insurance authorization submissions and ensure supporting documents are complete

    • Act as clinical liaison between the agency and payers for plan approvals and renewals

    • Support audit readiness and maintain compliance with Medicaid and state guidelines

    • Provide real-time support and feedback to providers around documentation and planning

    • Monitor trends and identify areas of risk or need for retraining

     

    Qualifications and Must-Haves:

    • experience working in a clinical HCBS or community-based setting
    • Strong documentation and compliance background

    • Proficient in writing and reviewing Medicaid-aligned service plans

    • Able to conduct staff training and support documentation audits

    • Familiarity with OMH/DOH regulations for HCBS services

    • Experience with EMRs or case management platforms preferred
