We are seeking a dedicated HCBS Service Coordinator to support individuals receiving Home and Community-Based Services. This role involves coordinating services, ensuring care plans are followed, and helping clients live independently in their communities.

Key Responsibilities:

• Coordinate and monitor HCBS services for assigned clients
• Develop and implement person-centered care plans
• Collaborate with interdisciplinary teams and service providers
• Maintain accurate documentation and compliance with state guidelines
• Advocate for the needs and rights of clients

Qualifications:

• Bachelor’s degree in health, human services, psychology, social work, or related field
• Experience working with individuals with behavioral health needs preferred
• Strong organizational and communication skills
• Familiarity with HCBS or case management is a plus
