As one of our Licensed Social Workers, you are responsible for the complete patient customer experience. This position reports to the Clinical Director. Under general supervision, this position is responsible for ensuring seamless care for patients by acting as liaison between patients and physicians.
LCSW collaborates with a multidisciplinary team. It is the primary responsibility of the LCSW to ensure that the most appropriate quality medical care is provided to members.Strong hospitality focus, organization, flexibility, time-management, and can-do attitude will provide day-to-day success. QUALIFICATIONS:
LICENSED SOCIAL WORKER (LCSW):
- Bilingual - English and Spanish
- Bachelor’s Degree in social work required
- Master's degree in Human Services
- Active unencumbered License to practice in Louisiana
- 2-3 year minimum experience in a health related setting
- At least 1yr of supervised experience in a healthcare setting working in direct patient care
- Valid Driver’s License, in good standing
- Car registration and insurance current
- CPR certified
- Crisis Intervention Certification
- Ability to plan, organize and implement social service programs for residents with varying needs
- Current knowledge of state and federal laws applicable to social work in mental and behavioral health
- Excellent interpersonal and communication skills
- Proficient with computers
- Must have a computer and internet access
RESPONSIBILITIES:
- Complete comprehensive psycho-social histories and Intake Assessments as assigned; meeting all compliance requirements for content and quality assurance
- File all compliance documentation, including but not limited to: Diagnostic Evaluations; Plan of Care Reviews; Master Treatment Plans; Suicide and Crisis Assessments and Planning; Urgent Coordination Notes; Mental Health Professional Progress Notes; and, other documentation as required
- Identify barriers and plans for intervention to overcome or lessen barriers to achieve outcome as evidenced by treatment plan. Communicates findings and plans to an interdisciplinary team. Documents assessment, plan and interventions in medical record. Conducts reassessment based on patient need and plan of care.
- Responsible for the timely submission of all required compliance documentation
- Supervise MHP case load
- Provide direct services including: Intake Assessments; Individual Therapy; Group Therapy; Collateral Intervention; Family Therapy; Crisis Stabilization / Intervention; Professional Intervention; Diagnostic Interpretation; and, other Medicaid or insurer authorized service codes
- Meet or exceed minimum weekly established or revised production goals for reimbursable billing related to direct service provision
- Initiate and coordinate emergency hospitalization as required and coordination of follow up care
- Coordinate efforts to provide mental health services in conjunction with other providers.
- Coordinate with the Clinical supervisor for client care relative to periodic consultation, annual review, medication management; and, other matters
- Maintain effective communication with clients, parents, guardians, and others involved in coordination of client care
- Meet all regulatory and licensure requirements mandating availability for emergency services as needed
- Maintain consistent reporting to Clinical Director
- Interface with departmental employees in the coordination of required documentation, communication, timelines, and other requirements
- Perform any other related duties as required or assigned
