    Position Description:

    Under the supervision of the Clinical Director, the Social Worker/Counselor/Therapist performs a variety of clinical and administrative work involving the delivery of mental health services to adults, children and adolescents in need of treatment and support. This position will work with a team of clinicians in the planning, implementation and performance of therapeutically oriented activities and programs for consumers receiving mental health and substance abuse treatment services. This position requires considerable knowledge of mental health disorders and treatment methods, as well as an understanding of the principles and techniques used in the treatment of mental health disorders.

    Duties and Responsibilities:

    1. Receives referrals of clients in need of mental health services and monitors client's behavior in relation to treatment plan goals and clients’ needs.
    2. Maintains a caseload of individuals with· personal social behavior problems including severe mental illness.
    3. Conducts assessments of client needs, actively participates in the development of the consumer's Service Plan development and makes appropriate documentation.
    4. Participates in various staff meetings and in-services and communicates with team members on observed behaviors and progress of consumers.
    5. Provides counseling, therapy, and/or psychotherapy to consumers and families as appropriate to the position; prepares reauthorization plans, treatment plans, discharge plans, and provides crisis intervention and emergency services as required.
    6. Provides and/or arranges for therapeutic interventions as appropriate for consumers in a crisis condition and for those with serious disturbance problems.
    7. Plans and facilitates community outreach and education activities and may serve as an HHCBS representative at community organizations.
    8. Establishes and maintains case files, referrals, and other related documents for the treatment of consumers; prepares related documents for legal proceedings; keeps abreast of consumer data to ensure appropriate treatment and care are delivered.
    9. Completes weekly progress reports during team meetings on status of clients.
    10. Follow-up with referral source as well as community linkages.
    11. Complete monthly reports on status of clients and forward to referral source of Department of Juvenile Justice/Department of Family & Children Services/ Georgia Crisis Access Line, etc.
    12. Ensure that all MICP/treatment plans of clients participating in the Core program are assigned by teams after being authorized and printed from the APS system.
    13. Attend mandatory staff meetings weekly.
    14. Attend monthly chart review as scheduled by the Quality Assurance Department.
    15. Follow all agency policies and procedures.
