    Job Description

     

    MAJOR FUNCTIONS:

     

    Under general supervision, plans, and implements, health education programs, workshops, and other associated activities related to family planning health education services. Collaborates with other health specialists to ascertain health needs, develop desirable health goals, and determine availability of professional health services at HJAHC. Develops and maintain cooperation between public, civic entities and HJAHC. Identify and provide assistance to Title X eligible individuals. Have a passion for community health promotion, for serving the medically underserved, and you understand how to communicate health topics in meaningful, culturally sensitive, and relevant ways.

     

    ESSENTIAL FUNCTIONS:

    Direct Service:

    Plans, develops, and designs health education and training programs related to family planning, including preparation, facilitation and follow-up.

    • Serves as a point of contact in area of expertise; refers clients to other support services, such as medical services, social services, etc. as appropriate.
    • Prepares and disseminates family planning educational and informational materials.
    • Maintain current knowledge base on family planning, pregnancy, STD & HIV related health issues.
    • Educate patients about health insurance application process, advocacy and empowerment.
    • Engages in outreach activities to local churches, high schools, colleges, and Community Based Organizations.
    • Attends/helps organize community workshops, trainings and health/resource fairs.
    • Prepares daily, weekly, and monthly reports on program participant’s profiles, scores, tabulation and statistics and submit to program director.
    • Call patients to reschedule appointments and follow up on missed appointments for the Family Planning Unit.
    • Provide case management follow up to family planning patients.
    • Provide, on phone or in person, accurate, reliable information regarding departmental programs.
    • Assure timeliness of services rendered to patients and look after their comfort while on premises.
    • Performs miscellaneous job-related duties as assigned within the job classification.

    Program Development and Maintenance:

    · Assists in project data collection, evaluation activities, and required reporting. Completes all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards; in addition, documentation complies with program/grant requirements.

    · Individually and/or with a team, plans, creates, and develops health education topic outlines and curriculums, incorporating various learning techniques and formats (i.e., group discussion, lectures, role-play, etc.). Reviews and assists in determining the linguistic and cultural appropriateness of materials, teaching aids, activities, methods, strategies, programs, etc. Review the efficacy of materials and programs and provides positive, action-oriented feedback. Assists with the translation and/or revisions of translated materials as appropriate.

    · Perform telephone client satisfaction surveys and other data collection as appropriate.

    · Participates in the development, implementation, and evaluation of special projects and/or pilot programs.

    · Establishes and maintains necessary records and files. Drafts correspondence in the course of official duties.

    · Collects, records, and analyzes significant program-related data; develops monthly statistical and programmatic reports.

    Staff Development:

    · Serves as a full member of the HJAHC clinical staff team

    · Participates fully in staff meetings and departmental meetings

    · Participates in professional and related meetings, conferences, and continuing education training as available; prepares reports as requested.

    · Keeps apprised of the developing areas of social services and provides colleagues with updates as appropriate.

    · Assists in Title X departmental tasks, including programmatic and infrastructure design, implementation, and evaluation

    · Learns and implements various types of electronic and/or manual recording and information systems used by the Center or its related units

    EDUCATION:

    · AA degree and a minimum of two years of experience on family planning health education topics and related field; Bachelor of Arts (BA) preferred.

    • 2 years of experience delivering health education, preferably in a community health setting.

    · Preferred Completion of Family Planning Health Worker Certification

    · Basic knowledge of MS office

    · Ability to handle multiple tasks and work in a busy environment within job classification only.

    · Basic understanding of government programs related to services provided.

    · Able to work and communicate effectively with people of diverse culture, education and economic backgrounds.

    PERSONAL CHARACTERISTICS:

    · Must be able to read, write, speak, understand and communicate in English (and preferably in Spanish) sufficiently to perform the duties of this position.
