    Overview:

    The HIM Coordinator gathers and communicates all relative information to external providers. HIM Coordinators serve as a liaison for patient resources. The HIMl Coordinator will often work closely with insurance companies. The HIM Coordinator must be able to take and follow through with delegated tasks and accountability.

    Duties and Responsibilities:

    • 2-3 years of hands-on healthcare experience in a clinical setting

    • Maintain ongoing tracking and appropriate documentation on referrals to promote team awareness and ensure patient safety. This tracking will use the EMR database.

    • Experience with EMRs (Electronic Medical Record) / Medical Records

    • Proficient with Medical Terminology

    • Obtain Prior Authorization and Referrals from Insurance Companies

    • Coordinate with staff to track the status of all providers reports on patients referred to hospitals, outside clinics and Specialty Providers and follow up for reports/records not received on referred patients.

    • Assemble information concerning patient's clinical background and referral needs. Per referral guidelines, provide appropriate clinical information to specialists.

    • Review details and expectations about the referral with patients.

    • Assist patients in problem solving potential issues related to the health care system, financial or social barriers.

    • Identify and utilize cultural and community resources. Establish and maintain relationships with identified service providers.

    • Fax referral form to providers.

    • Respond to In-house provider questions, requests, and concerns regarding the status of patient referrals, care coordination or follow-up status.

    • Answer, respond and document phone calls, requests, and questions from patients in a timely manner. Calls must be accurately managed or redirected as appropriate.

    • Ensure that referrals are addressed in a timely manner. See “Internal Referral Tracking Policy”.

    • Routinely provide patients with other clinic and community-based resource materials as appropriate.

    • Other duties as assigned.

    Education, Licensure/Certification:

    • Must have a high school diploma or equivalent.

    Experience, Skills/Abilities Related Requirements:

    • Must have professional manners when functioning in a high public contact.
    • Must have aptitude to pay high attention to detail with the ability to navigate, compromise and negotiate successful tracking and coordination of referrals.
    • Must be able to work using independent judgment, take initiative in the solving of complex issues, planning, development and coordination of clerical support programs, and use of computer systems.
    • Must be able to work with discretion and communicate effectively when dealing with client complaints and protected information.
    • Must have excellent organization, prioritization of tasks, and interpersonal skills.

    Continuing Education and Training Requirements:

    • Participates in trainings required by the agency, funding source(s), and/or as required by licensure if applicable.
