Job Description
The HIV Integrated PrEP Navigator is a dedicated advocate and support professional who assists individuals at-risk of developing HIV or living with HIV in navigating the complexities of healthcare and social services. The role focuses on enhancing the quality of life for patients by providing comprehensive support, including healthcare coordination, education, and linkage to essential services. The PrEP Navigator will assess the needs of at-risk patients and refer them to Preexposure Prophylaxis (PrEP) and Post-exposure Prophylaxis (PEP) Services. The Navigator works collaboratively with healthcare providers, pharmacies, insurance, Ryan White Part A, social service agencies, and community organizations to ensure patients receive timely, appropriate, and holistic care.
KEY ACCOUNTABILITIES:
- Linking patients with HIV preventative and treatment service through follow-up on medical appointments, adherence education and groups support.
- Works collaboratively with the Clinical Case Manager and Integrated Behavioral Health Consultant to bring support services to the client.
- The HIV Integrated Care PrEP Navigator is responsible for conducting community HIV testing events and providing referrals for prevention services.
- Flexible work hours may be required during evenings and weekends.
- Support the organization as needed.
KNOWLEDGE, SKILLS, AND ABILITIES

- Knowledge of the cultural and social characteristics of the Native American and ethnic minority populations in Maricopa County.
- Knowledge of the economic and health constructs for Native Americans and ethnic/ sexual minority populations in Maricopa County.
- Knowledge and experience working with HIV positive and at-risk communities for HIV.
- Culturally responsive to ethnic and sexual minorities.
- Comfortable talking about safer sex practices and familiar with harm reduction best practices.
- Familiarity and relationship with HIV providers in Maricopa County.
- Experience conducting Rapid HIV testing or its equivalent.
- Experience drawing blood or willingness to become a certified Phlebotomist.
- Must possess excellent Communication skills.
- Bilingual Spanish speaker preferred.

- Must be able to work in an outreach setting and support mobile unit activities as need which includes lifting 15 lbs above your head.
- Be detailed oriented professional and effective leader who can work well with other health care personnel and interact in a culturally responsive way with patients and their families.
- Knowledge of client confidentiality and client rights according to Health Insurance Portability and Accountability Act (HIPAA).
- Ability to effectively function as a team member as part of Native Health.
