    Home Care Support Associate works with Client Care Coordinator to provide excellent customer service for customers utilizing our Home Care Partner home care services. The Home Care Support Associate is responsible to coordinate and maintain staff scheduling for Professional Nurse Partners Home Care clients and staff. The Home Care Support Associate agrees to “float” as needed as a Home Care Partner providing in home care support to our clients. The Caregiver Floater works with a variety of different clients who have varying shift needs. This position is guaranteed full time hours. Some work in the office filing and performing other office duties between shifts with clients may be assigned. Willingness to travel to clients’ homes in all service areas is required. Performs personal care activities that assist the client with activities of daily living.

    Responsibilities/Activities

    Ensure Professional Nurse Partners Home Care Partner policies and procedures and industry standards and regulations are followed.

    Creates and maintains staff schedules on a daily/weekly/monthly basis

    Reach out to personnel and clients with regards to changes in schedule that may come up on a daily basis

    Provides staffing for sick leave, vacation, and unplanned call outs.

    Enters scheduling data, creates schedules and distributes to staff as appropriate.

    Contacts care staff and clients regarding day-to-day changes in scheduling.

    Assist with the coordination, evaluation and planning of client care services.

    Coordinate client staffing needs.

    Enters staff and client information into organization’s database.

    Maintains staff and client database, dates and reminders of appointments, client admission, client hospital admission and discharge. ¨ Develop and instruct employees in the use of practices, procedures and equipment.

    Provide leadership to employees.

    Conduct in-home safety assessments as needed and on on-ongoing basis.

    Performs general office duties including but not limited to word processing, photocopies, files, shreds, data input, provides reception and telephone services.

    Required Knowledge

    Knowledge of standards of practice and scope of practice, in accordance with state regulations ¨ Comprehensive knowledge of other health care disciplines and their role in client and family care.

    Knowledge of personal care and home management skills.

    Knowledge of principles and processes for providing client and personal care services, including needs determinants, meeting quality standards and evaluation of client satisfaction.

    Knowledge of clerical procedures such as maintaining records and completing forms.

    Qualifications/Education

    Current registration on State of California Home Care Aide Registry

    High School Diploma

    Current driver’s license.

    Proper Vehicle Insurance Coverage.

    Ability to read, write, speak and understand English as needed for the job.

    Possess or obtain a valid TB test

    2 years recent, valid, verifiable work experience

    Six months experience as a Personal Care Assistant in healthcare (in homes or facilities) is preferred.

    CNA or HHA license is preferred
