Home Health Care Social Worker Responsibilities & Duties

Provide emotional and practical support to patients and their families in their homes

Assess patients' needs and develop individualized care plans

Link patients with community resources and services

Collaborate with interdisciplinary healthcare teams to provide holistic care

Maintain accurate and up-to-date patient records

Stay current with advancements in home health care and social work practices

Home Health Care Social Worker Qualifications & Skills

Master's degree in Social Work (MSW)

Licensed Clinical Social Worker (LCSW) certification

Experience in health care or home health care settings

Strong understanding of health care systems and policies

Proficiency in electronic health records (EHR) systems

Fluency in multiple languages

Advanced training in geriatric care or chronic illness management

Excellent interpersonal and communication skills

Ability to work independently and manage a caseload

Bachelor's or Master's degree in Social Work

Licensed in good standing as a social worker

2+ years of experience in home health care or related field

Excellent communication and interpersonal skills

Strong organizational and problem-solving skills

A commitment to patient-centered care and advocacy

Previous Home health experience that works with Medicare, Medicaid, and/or VA patients
