    POSITION OVERVIEW:

    The Registered Nurse (RN) promotes and restores patients' health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families.

    PRINCIPAL DUTIES & RESPONSIBILITIES:

    • Identifies patient care requirements by establishing personal rapport with potential and actual patients and other persons in a position to understand care requirements.
    • Conduct triage of patients, provides HIV and Hepatitis C testing.
    • Conducts street outreach and participates in community/agency outreach events to engage potential clients into services.
    • Collect laboratory specimens in accordance to Federal law, regulators agencies and HHCLA policies and procedures.
    • Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.
    • Promotes patient's independence by establishing patient care goals; teaching patient, friends, and family to understand condition, medications, and self-care skills; answering questions.
    • Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following hospital and nursing division's philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations.
    • Maintains safe and clean working environment by complying with procedures, rules, and regulations; calling for assistance from health care support personnel.
    • Protects patients and employees by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations.
    • Ensures operation of equipment by completing preventive maintenance requirements; following manufacturer's instructions; troubleshooting malfunctions; calling for repairs; maintaining equipment inventories; evaluating new equipment and techniques.
    • Maintains medical supplies inventory by checking stock to determine inventory level; anticipating needed supplies; placing and expediting orders for supplies; verifying receipt of supplies; using equipment and supplies as needed to accomplish job results.
    • Complies with federal, state, and local legal and professional requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actions.
    • Assists with developing, updating and implementing medical policies and procedures.
    • Complies with weekly productivity requirements to ensure compliance with contractual and performance management requirements.
    • Implements and consistently practices harm reduction, trauma informed care, housing first and integrated care interventions.
    • Able to serve a diverse vulnerable client populations and communicate effectively in a culturally competent manner.
    • Able to work in a multi-disciplinary team environment, utilizing professional, open and respectful communication. Maintains a solution focused, strengths based and client centered approach with managers, co-workers and clients of HHCLA.
    • Ensures performance management standards are met through ongoing supervision addressing barriers as they arise.
    • Maintains client records according to HIPAA and adheres to all client confidentiality requirements and standards.
    • Complies and maintains familiarity with agency and program policies and procedures and other applicable regulations.
    • Attends and participates in supervision, staff meetings, trainings, conferences, workshops, and special projects to promote professional development.
    • Models HHCLA approach, mission and core values in all communication, correspondence, community events, coalitions and advocacy efforts.
    • Reports to work on time and maintains reliable and regular attendance.
    • Maintains current licensing and/or certification as required for position.
    • Other duties as assigned.
