POSITION SUMMARY
This position performs duties under the general direction of the Habilitation Coordinator. The person Is responsible for assessing life skills, developing community integrated opportunities and providing in-home supports and life skills training for persons with disabilities. Develop opportunities, in consultation with participants, and perform requested Individualized habilitation services for assigned participants.

Essential Functions Statement(s)
• Participates in the habilitation services referral process, including gathering information, notifying appropriate parties, and developing habilitation plans.
• Provides a thorough assessment of Individuals needs, interests and skills by using a variety of tools and methods to support life skills training.
• Supports and supervises pre-vocational activities which may Include such activities as volunteering, exploring interests, hobbies, and skills.
• Provide all necessary written documentation required by various licensing entitles and funding sources.
• Provide on-site life skills training and follow-up services depending on the needs of those served.
• Provide in-home support with life skills to ensure progress in goals from individual support plan.
• Assist and arrange for support of volunteer activities for QUEST participants as requested.
• Represent QUEST In the best possible manner throughout all community transactions to increase awareness around community integration options for people with disabilities.
• Document and track work time dedicated to supporting Individuals, travel, and personal staff training.
• Participate In and help to support fund raising activities of the agency.
• Refer participants to community resources for services such as debt counseling, legal aid, housing, and medical treatment or financial assistance.
• Provide individualized support as requested by participants, family and Supports Coordinator In the community.
• Evaluate personal characteristics and home conditions to provide support in the areas needed.
• Develop and review service plans in consultation with participants and their team.
• Attend staff meetings required.
• Perform other related duties as assigned to support across programs and the organization.
SKILLS & ABILITIES
| Education: | Bachelors Degree (4 year college or technical school) or Associates Degree: Preferred. Highschool or GED: Required | Experience: | At least 1 year in Human Services or related field | Computer Skills: | Microsoft Suite, resume creation, basic computer skills are necessary. | Certifications & Licenses: | NA | Other Requirements: | Valid Driver’s License, clean driving history, and reliable personal vehicle for transportation of participants.
WORK ENVIRONMENT
Work will be frequently conducted in the community and in participants homes. Noise levels will also vary depending on the job location.
