    PURPOSE OF POSITION

    The Health Navigator will provide ongoing case management and substance misuse support to members of the LGBTQ+ community. The Health Navigator will facilitate a multi-pronged intervention consisting of sexual health, brief interventions, and education sessions. They will work as a part of an interdisciplinary team, identifying internal and external referrals as appropriate.

    RESPONSIBILITIES:

    The major responsibility of this position includes, but are not limited to:

    Conduct HIV/HCV testing and risk assessments.

    Work with social media team to develop marketing campaigns for priority population.

    Provide referrals for STI testing.

    Link clients lost to care to HIV primary care services.

    Provide ongoing HIV Navigation Services to a minimum of 75 clients per contract year.

    Advocate for clients' needs and rights within the healthcare system, ensuring they receive appropriate care and support.

    Conduct comprehensive assessments of clients' healthcare needs and provide guidance and support to address those needs effectively.

    Assist clients in navigating the healthcare system, including scheduling appointments, obtaining referrals, and accessing necessary services including SUD treatment or Harm Reduction services.

    Educate clients about their health conditions, treatment options, and available resources to empower them to make informed decisions about their healthcare.

    Coordinate care among various healthcare providers and services to ensure seamless transitions and continuity of care for patients.

    Connect clients with community resources, support services, and financial assistance programs to address social determinants of health and improve overall well-being.

    Facilitate evidence-based interventions such as 3MV, RESPECT & Dup!

    Demonstrate cultural competency and sensitivity in working with diverse populations, respecting clients' cultural beliefs, values, and preferences.

    Maintain accurate and up-to-date records of client interactions, services provided, and outcomes achieved. Generate reports as needed to track program effectiveness and outcomes.

    Work with outreach team to identify recruitment venues in which the priority population congregates.

    Manage day-to-day Drop-in Center/office activities that incorporate social, education, and community building activities.

    Must meet at least 85% of program deliverables.

    Ensure all data and reports are submitted by the prescribed deadline.

    Will provide support and mentorship to the Outreach Specialist and Peer Educators, in the form of trainings and individual development, maintaining cultural humility.

    Must remain abreast of people-first language, serving as an intermediary for the organization.

    The Health Navigator may work in a variety of settings, including hospitals, clinics, community health centers, and social service agencies.

    Rotating weekends are required.

    Other duties and responsibilities can be assigned as necessary.

    EDUCATION

    Associate Degree or higher in relevant field and equivalent practical experience required.

    QUALIFICATIONS:

    Minimum of three (3) years of similar work experience, facilitating groups and performing outreach/community engagement activities.

    Certified in Rapid HIV/HCV Testing

    Previous experience delivering evidence-based interventions such as 3MV, RESPRCT & D-Up!

    Previous work experience with high risk, YMSM, HIV+ populations.

    Work experience with LGBTQ+ individuals in a medical, health or social service setting.

    Knowledge of citywide resources and referral processes for LGBTQ+ individuals.

    LGBTQ+ life experience.

    Outstanding writing and communication skills; including multi-media outlets.

    Previous experience in providing healthcare navigation, care coordination, or patient advocacy in community-based medical and/or social service agencies.

    Strong communication and interpersonal skills.

    Excellent organizational and time management skills.

    Proficient with Microsoft Office Suite and related programs

    Compensation & Benefits**

    The base pay range for this position is $43,000-$53,000 per year. *
