    PURPOSE OF POSITION

    The Case Manager will conduct intake interviews, care plan development, care coordination, referral services, substance use counseling coordination, health education, and social services navigation to the priority population. . The Case Manager will act as an intermediary between providers and members – serving in numerous roles such as educator, evaluator, service coordinator, community resource researcher, and more.

    RESPONSIBILITIES:

    The major responsibility of this position includes, but are not limited to:

    Completes initial assessment as needed and reassessments of all assigned participants in a timely manner in-order to develop individualized treatment goals for each participant.

    Establishes treatment goals and objectives with participants as part of the interdisciplinary team.

    Provides case management services including but not limited to weekly and monthly progress summaries, quarterly assessments, quarterly care planning, counseling, outreach, case conferences, advocacy, referrals (on-site & off-site) and discharge planning.

    Ensure timely submission of required documentation to social support organizations for continuity of services.

    Provide referrals for shelters and transitional housing, along with other resources as needed.

    Coordinate transportation services as needed.

    Educate participants on basic life skills.

    Work collaboratively with team members to provide outreach and engagement too-hard-to-reach participants.

    Maintain documentation and update care plans at specific time intervals or as needed in designated medical records systems (.EClinicalWorks and Health Homes EMR)

    Assists participants with navigating the health insurance system including the AIDS Drug Assistance Program (ADAP)

    Must meet at least 95% of program deliverables.

    Understands and is sensitive to chronic medical conditions, including HIV, mental health, substance use, diabetes, asthma, hypertension, cardiovascular health, STIs, etc.

    Other duties and responsibilities may be assigned as necessary.

    EDUCATION: High School Diploma or GED, and at least two years of applicable experience and/or equivalent experience.

    QUALIFICATIONS:

    Previous professional or personal experience in housing placement and assistance preferred.

    Previous work experience with unhoused, underserved, and disproportionately-affected communities.

    Outstanding writing and communication skills; including multi-media outlets.

    Proficient with Microsoft Office Suite and related programs.

    Advocacy and interpersonal skills.

    ADDITIONAL QUALIFICATIONS:

    Prior work experience with unhoused individuals in a medical/health or social service setting.

    Previous experience in providing healthcare navigation, care coordination, or patient advocacy in community-based medical and/or social service agencies.

    Knowledge of citywide resources and referral processes for unhoused individuals

    Bilingual in English/Spanish a plus

    Compensation & Benefits**

    The base pay range for this position is $40,000-$45,000/year. *
