  • Responsibilities

    Direct Care Associate:

    Position Description:

    Work closely with the individuals participating in Hands United programs to assist in achieving and maintaining their goals on their Individual Service Plans.

    Specific Responsibilities of the Job:

    Meet the ongoing care and instructional needs of individuals with intellectual disabilities and mental health needs.

    Minimize risks of endangerment to the health, safety, and well-being of the individual participant

    Establish and maintain a constructive and professional relationship with the individual and their circle of support

    Communicate daily with the direct supervisor regarding any changes or issues related to the individual participant

    Major Areas of Responsibility:

    Transport individuals to medical, dental, social activities, and other appointments.

    Assist individuals in all personal grooming such as bathing, toileting, dressing, medical, and dental needs.

    Assist with coordination of individuals healthcare.

    Provide supervision and training during community and recreational outings.

    Monitor physical and emotional wellness of individuals and immediately report changes in status.

    Prepare meals and store food in compliance with all regulations and the program policy.

    Follow response procedures for all emergencies (medical, natural disaster, fire and behavioral incidents),

    Assist with maintenance and sanitation of home and surroundings.

    Record data and maintain files regarding individuals’ health, progress, and special incidents as outlined in Title 17, and the Hands United program.

    Assist in maintenance of administrative records, individual files, inventories and weekly schedules as needed.

    Advocate on behalf of the individual; uphold respectful and efficient communication with participant and all relevant parties. Understand organization and support systems relating to the individual’s life.

    Attend all required trainings and staff meetings.

    Must be able to work at diverse and unpredictable job sites for prolonged periods of time without assistance or direct supervision

    Required Knowledge, Skills, and Abilities:

    Prior experience providing direct care and supervision to persons with intellectual disabilities is preferred.

    Excellent communication skills; respectful and positive attitude toward individuals, coworkers, contractors and home visitors.

    Ability to complete training of nonphysical methods of crisis intervention within the first 45 days of employment.

    Ability to complete required staff training at a minimum of 4 hours each month.

    Ability to complete orientation and on the job training within the first two weeks of employment.

    Ability to meet Title 17 requirements including, but not limited to:

    Minimum age of 18

    High school diploma or GED certificate

    Health screen and TB clearance

    Live scan fingerprint and a clear criminal record

    CPR and first aid training and administration

    Valid and current California driver’s license

    Clear driving record

    Ability to lift up to 75 pounds

    Some weekend, evening and overnight shifts are required

    Maintain current and valid licenses, certificates, or registrations that may be legally required to provide the service

    Completion of annual training as required by Title 17, 58652

    Education and Experience:

    High school education or GED
