    Job Description

     

    IMMEDIATE SUPERVISOR: Clinical Director

    DESCRIPTION : This position provides direct services to clients and families. The candidate will prove quality assurance checks ie reviewing, editing and updating of client files for all programs, initial assessment, linking and coordinating services to families. Additionally the candidate will participate in regular meetings and weekly supervision and perform the task outline below.

    TASKS :

    Complete initial assessment of client/family situations and ongoing monthly reports following the approved format. Participates in the service plan within the initial thirty-day period

    • Do all prior authorizations and documentation, Monitor length of client services and notify referral sources prior to the completion of ninety-day period to determine if continuation is necessary, conduct initial Medicaid eligibility checks.
    • Have weekly contact with the Referring Agency.
    • Arrange for weekly case supervision with clinical supervisor.
    • Conduct quality Assurance and customer satisfaction survey, Audit charts weekly to assure that information is updated and accurate,
    • Act as the liaison between family, referring agencies, and other community organizations.
    • Coordinate client services between various agencies.
    • Maintain case records and document services rendered.
    • Attend weekly staff meetings consistently. Prior approval from management team is required when absent.
    • Documentation of group supervision notes, minutes and other trainings for staff
    • Provide services to a minimal of two families and facilitate connections of counselors to families after initial assessment to assure seamless transition of services.
    • Provide general program and administrative support to all Hope programs.
    • Prior to the end of each Ninety-day service period, coordinate and facilitate a staffing for the client, inviting the family of the client, the Referral Source, representation from other pertinent agencies providing services and supervision.
    • Maintain high degree of client confidentiality.
    • Work with other staff assigned to cases, communicating effectively.
    • Maintain a high degree of professionalism in the community, with clients, and with referring agencies seeking to build and maintain positive relationships.
    • Assist in providing Individual and Family counseling services to clients in the home and community setting.
    • Assist in providing Crisis Intervention as needed.
    • And all other duties assigned by supervisor or executive member.

    ADDITIONAL DUTIES THAT ARE IMPORTANT TO THIS PARTICULAR POSITION:

    • Take the majority of referral calls.
    • Request add'l referral information (if from the hospital) and/or fill out a referral statement with pertinent information.
    • Check Medicaid Status on Medicaid and Magellan Website. (Magellan has a recent address which is an important piece of information). Note: which type of insurance do they have to determine expected stay.
    • Maintain frequent contact with referral sources regarding the status of referral (i.e. if we have room or not). This includes making contact with Director of Divi in order to inform her of the referral, checking availability, and making sure that she believes the potential client is an appropriate referral.
    • Schedule and complete preliminary assessments with potential clients (For Crisis: if they are in the hospital)(For MHSS: Schedule the full assessment with the client in their home at a later time) to determine eligibility.
    • Complete assessment interview and/or intake process (I frequently take incoming crisis clients to the pharmacy to drop off medications and will sometimes take them to the hotel to get setup there).
    • Complete Initial ISP before the end of the assessment interview with crisis client. (complete comprehensive ISP later on(this can be done at the intake interview if times allows)). The comprehensive ISP is due within a 72-hour window.
    • Complete Assessment Documents and accompanying progress note.
    • Send in Request for Registration (for Crisis) and Request for Authorization (for MHSS).
    • Email or give all assessment documents to who needs it (i.e. Oliver (for signature), Allen, Powell, and identified counselor.
    • Communicate with insurance and Keisha regarding authorization status. (Provide Keisha with authorization approval info when approved)
    • Complete Master ISPs and Biopsychosocial Assessment for MHSS Clients (Must be completed within 30 days of the start of MHSB services).
    • Complete Weekly referral report and attend weekly management meetings to present report.

    MINIMUM QUALIFICATIONS:

    A Bachelor's Degree with 2 years of successful work history and experience working with children and adolescents and families with mental illness and meets the criterion for services.  A Masters Degree and Licensed or Licensed Eligible as defined by DBDHS and 2 years of successful work history and experience working with children and adolescents and families with mental illness and meets the criterion for services, is preferred. The successful candidate will need to have proof of completion of an approved human services related program.
