    Job Overview

    Provide health monitoring, screening, and supervision for frail elderly and elderly with dementia in an adult day program.

    Responsibilities and Duties

    · Perform health screening

    · Monitor MD orders

    · Assess potential and current clients

    · Supervises clinical students

    · Prepare care plans for frail and isolated seniors.

    · Supervise medication administration and personal care of clients, including meal assistance, walking, transfers, and bathroom use. Assist clients as needed.

    · Report client status to, and plan care with, caregivers and professional contacts.

    · Provide educational programs for staff, clients, and families.

    · Other reasonable duties as assigned and any other duties that may be required of you within your scope of practice.

    Requirements:

    · 18 years of age or older.

    · Valid Registered Nursing License through the state of Indiana with no pending or active violations on Licenses. The selected candidate will be required to submit proof, in the form of official transcripts, of any and all required education attained by the applicant, which is indicated on the application and/or resume prior to the date of hire.

    · Post-high school education is advantageous.

    · Previous experience in an adult day care or similar setting

    · Proficient in Microsoft Office applications (Word, Excel, Outlook).

    · The ability to type and file accurately.

    · Excellent communication, leadership, interpersonal, and organizational skills.

    · Good telephone etiquette. To be successful as a scheduler, you should be helpful and courteous when answering telephone calls. Ultimately, an exceptional scheduler should be able to build and maintain a good rapport with referring physicians and staff.

    · Knowledge of state and federal regulations for adult day care centers

    · Ability to handle stressful situations with patience and professionalism

    Education and Experience

    · Valid Registered Nursing License (ASN or BSN)

    · Business college training an advantage.

    · Previous RN experience may be requested, but this can also be an entry-level position.

    · Competent computer skills, including MS Office

    · Internet skills, including use of e-mails, group messaging, and data collection.

    · Proficiency and literacy skills
