Licensed Practical Nurse (LPN/LVN) / Care Navigator
The Licensed Practical/Vocational Nurse as a Care Navigator performs interactive telephonic outreach to provide medication therapy consultations to address drug therapy problems related to medication safety and use, medication omissions in care, and barriers to medication adherence.
The associate is responsible for the functions below, in addition to other duties as assigned:
Handling patient calls such as medication review, reconciliation and adherence for a variety of clients and programs to meet performance and quality standards and program goals using clinical and medication expertise and critical thinking skills
Engage patients through motivational interviewing to assist patients in changing behaviors related to medication use and adherence
Accurately complete all electronic documentation to ensure compliance with company standards, program goals, guidelines and government requirements
Collaborate with healthcare providers on patient’s behalf to address drug therapy problems
Participate in required trainings as assigned and support security and compliance related activities and task throughout execution of all job duties
QUALIFICATION REQUIREMENTS
To perform this job successfully, the associate must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skills, and/or abilities required:
High School Diploma or General Education Degree (GED); Associate’s degree (AA) Nursing preferred
Graduate of LPN/LVN diploma or certificate program required
2 years of experience working in a clinical setting as an LPN/LVN required
1 year of experience in Working in an office setting, call center setting or phone support role preferred
1 year of experience in Working in case management preferred
Active Practical or Vocational Nurse (LPN/LVN) license in good standing in state of residence and/or work location; with ability to obtain and maintain multi-state licenses as dictated by business needs required
Reside in a state that is part of the Nurse Licensure Compact and hold multi-state privileges in good standing
Sound clinical judgment
Ability to use pharmaceutical references
General knowledge of therapeutic categories and common indications of medications
Technology savvy; experience using Windows-based software including the Microsoft Office suite
Excellent verbal and written skills in English; fluent in Spanish a plus
Ability and willingness to move with purpose and a strong sense of urgency
Display a high degree of dependability in personal attendance and punctuality
Medicare Part D knowledge preferred
SCHEDULE
Candidates must be able to work the schedule of 10:00 AM to 6:30 PM Eastern Time (8 AM to 4:30 PM Mountain) Monday – Friday. Saturdays may be necessary based on business needs.
Full-time, regular role
LOCATION
This position is a home-based remote position. Candidates will need to meet Health Dialog’s remote working requirements. Candidate must live in a state that is part of the Nurse Licensure Compact and hold multi-state privileges in good standing
