    Job Description

    JOB DESCRIPTION: Process patient referrals as received from primary care physician to specialty office in accordance with external referral policy. Communicate appointment status to patients, and follow-up as necessary. Communicate appointment status to primary care physician.

     

    ESSENTIAL DUTIES AND RESPONSIBILITIES:

    • Demonstrates effective communication and problem solving skills. 
    • Responsible for the organization and completion of referrals in a timely manner.
    • Maintains daily electronic tracking of all referrals received and follow ups, enters data into EMR System (i.e., pending appointments, appointment letters, pending consults, referrals, etc.).
    • Provides daily updates to the Center Manager and/or Supervisor regarding referral status.
    • Completion of follow-up on all referrals including documentation in EMR and tracking system, rescheduling appointments and notification of the PCP.
    • Notification and follow-up with the patient and provider specialist of all necessary information for patient’s appointment.
    • Ongoing knowledge, documentation and update of appropriate referral resources, required forms and key contact persons.
    • Collect and maintain required data for any corporate requirements.
    • May be required to interpret for patients and staff on an as needed basis.
    • Insures client patient confidentiality.
    • Other duties and/or special projects as required.

     

    MINIMUM QUALIFICATIONS: 

    KNOWLEDGE: Bilingual Spanish/English

    SKILLS: Clerical and computer skills (general knowledge of office procedures and the ability to work with computer systems). Medical office, medical insurance billing, medical terminology or referral experience.

    ABILITY TO: work effectively and professionally with staff, patients and the public; strong interpersonal/communication skills. Ability to handle confidential information.

     

    EDUCATION/EXPERIENCE REQUIREMENTS:

    • High school graduate or equivalent. 
    • Clerical and computer skills (general knowledge of office procedures and the ability to work with computer systems). 
    • Medical office, medical terminology, medical insurance billing or referral experience.
