    HEALTHFLEX HOME HEALTH SERVICES is actively seeking an experienced MEDICAL RECORDS CLERK for our OAKLAND OFFICE. By joining our team, you will assist in the coordination of patient care to help ensure that our community will get the quality home health care they need to improve their quality of life and well-being, one person at a time.

    SUMMARY OF JOB: Assist in the coordination of patient care under the director of the Operations Manager and Care Team. RESPONSIBILITIES

    • Send, receive, upload and maintain physician order in agency EMR system.
    • Assist in answering incoming phone calls and route messages as needed.
    • Create charts for new patients with proper filing of documents.
    • Perform scanning of consult notes, labs, progress notes etc
    • Follow up with Case Managers for hospitalized patients and send required documents to facilities.
    • Print orders for Account Executives to take to physicians for signatures.
    • Call patients or physician offices when information may be missing in patient's charts.
    • Assist with DME coordination for patients per Doctor's orders.
    • Verify PECOS enrollment for new physicians.
    • Send requests to Medical Records for resumption for care post hospital discharge.
    • Handle difficult and/or sensitive matters with careful judgement and tact on consistent basis and advising supervisor of unresolved issues.
    • Assist with clerical tasks such as photocopying, faxing, mailing, sorting/distributing of documents.
    • Ensures patient confidentiality is met by respecting HIPAA rules and regulations.
    • Contacting patients based on contact protocols.

    QUALIFICATIONS

    • 1+ years experience in a Home Health Care setting strongly preferred

    • 1+ years experience in Medical Records strongly preferred

    • Positive attitude and superb customer service skills

    • Organized and able to multitask in a busy environment

    • Goal oriented, proactive and takes initiative

    • Team player
