    HealthFlex is seeking a Home Health LVN Clinical Coordinator to join our team for our Oakland office and ensure that patient care is coordinated and managed appropriately.     RESPONSIBILITIES:

    • Actively involved in the daily operations and interacts with front line clinicians on a routine basis.
    • Provides clinical education, including mandatory requirements (in the field and off the field)
    • Facilitates case conferences
    • Responds to patient complaints and follows up as necessary with clinicians
    • Referral review prior to acceptance as needed
    • Involved in clinical oversight and documentation submission process
    • Implementing communication process with clinicians (includes K-Mail, secure texting, and cell phones/office calls)
    • Provides guidelines for standard care plans for frequent diagnoses & properly assessing which patients are high risk for hospitalization
    • Schedule guidelines for clinicians and proper practice of time management
    • Participate in quarterly QAPI meetings and initiatives to incorporate clinical record reviews, trends & surveys
    • Identify areas for improvement and implement PIPS 
    • Engage others in QAPI projects - encourage feedback 
    • Track and trend appropriate clinical data to ensure quality initiative are met
    • Prepare and present monthly inservices and educational meetings with clinical team in compliance with Medicare regulations
    • Ensures timely responsiveness to all clinical matters through various channels of communication
    • Review weekly reporting of delayed charting, proper care and documentation of wound care and high risk patients  
    • Create organized tracking method of wound care and high risk patients to ensure appropriate plan of care and treatment
    • Monitoring and tracking infection control, adverse events, and incidents reports and incorporating into QAPI initiatives
    • Additional responsibilities as assigned by Director in relation to clinical, operational, and educational matters
    • Education:
      • Ensures promotion and control of optimal patient care using outcome based and best-practice care planning
      • Provie OASIS Education to clinicians as needed, promoting point of care charting, timely/accurate documentation, and OASIS competency.
      • Educating clinicians on how  to formulate an individualized Plan of Care. Plan of care will then be reviewed along with Frequency and Duration.
      • Education of software/hardware, optimal time management, competency in physical assessment technique and accurate/timely data capture, including OASIS process measures with related care planning
      • Investigates and works collaboratively with physician groups to evolve continuum-based protocols for commonly served diseases
        • Help integrate advancing technology, such as telehealth and help with servicing in end of life care.
      • Following HEALTHFLEX Home Health Scope of Practice
      • Knowledge of the agency's adverse events (specifically emergent care and hospitalization of patients) and collaborating with Clinical Operations team

    REQUIREMENTS:

    • Current California Licensed Vocational Nurse license.
    • Minimum of 2 years of Home Health LVN experience
    • Knowledge and experience in coordination of care in home health
    • Excellent oral and written communication skills with leadership experience
