POSITION SUMMARY: HealthFlex is seeking a compassionate and experienced Home Health Physical Therapist (PT). By joining our 5 star rated clinical team; you will be providing skilled services to help patients regain their independence in the comfort of their own home.

SCHEDULE: Full Time, Part Time or Per Diem

LOCATIONS: We have needs across all of our territories in the following counties, Alameda, Contra Costa, Santa Clara, San Mateo, San Francisco, Marin, Sonoma, Napa and Solano. 

RESPONSIBILITIES:

• Perform evaluation in the home and develop a care plan.
• Help patients with mobility, movement, and help regaining their independence.
• Create detailed treatment and care plans for each patient.
• Communicate and coordinate with patients' physicians to ensure a quality level of care.
• Evaluate and report patients' progress.

QUALIFICATIONS:

• Current and unrestricted California PT license required.
• Current AHA CPR certification
• Valid Driver's License
