    Help Us Grow Home Care in South Bay San Diego

    HomeWell Care Services is seeking an experienced, outgoing, and results-driven Healthcare Community Liaison to develop referral relationships and connect local families with high-quality non-medical in-home care.

    We need someone who enjoys being out in the community building relationships—not sitting behind a desk.

    You will regularly visit hospitals, skilled nursing and rehabilitation facilities, senior living communities, physician offices, hospice and home health organizations, senior centers, and other referral partners throughout South Bay San Diego.

    Your goal is straightforward: build trusted relationships that generate qualified referrals and new clients for HomeWell Care Services.

    What You'll Do

    Build relationships with healthcare and senior-care referral partners.

    Conduct consistent in-person visits to hospitals, SNFs, rehabilitation facilities, assisted living, memory care, physician offices, hospice and home health organizations.

    Develop relationships with discharge planners, case managers, social workers, nurses, senior-living professionals, and community leaders.

    Generate qualified private-pay home-care referrals.

    Schedule one-on-one meetings, presentations, and lunch-and-learns.

    Follow up consistently with referral sources and prospective clients.

    Identify new referral opportunities throughout South Bay.

    Track contacts, visits, referrals, and results in our CRM.

    Coordinate with our Intake Coordinator and operations team to help convert qualified referrals into successful starts of care.

    Meet weekly outreach and monthly business-development goals.

    Who We're Looking For

    Healthcare or senior-care business development experience is strongly preferred.

    Ideal candidates may have experience in:

    Home care or home health

    Hospice

    Hospitals/discharge planning

    Skilled nursing and rehabilitation

    Assisted living or memory care

    Senior services

    Healthcare or medical sales

    Qualifications

    2+ years of healthcare business development, community outreach, referral development, or relationship-based sales experience preferred.

    Knowledge of the San Diego healthcare/senior-care community strongly preferred.

    Existing relationships with local healthcare referral sources are a major plus.

    Confident developing new relationships from the ground up.

    Strong communication, presentation, organization, and follow-up skills.

    Self-motivated and comfortable being accountable for measurable results.

    Comfortable using a CRM.

    Valid California driver's license, reliable transportation, and auto insurance required.

    A bachelor's degree is not required. We value relevant experience, relationships, performance, and results.

    Compensation & Opportunity
