**We are currently looking for a Licensed or Associate Licensed (Social Worker, Marriage and Family Therapist, or Professional Counselor) to serve as the Assistant Lead for our ACT Team.

_ACT (Assertive Community Treatment) is a highly intensive, outreach-based, out-of-clinic, service provided by a multidisciplinary team of professionals. The population targeted is individuals who have serious and persistent mental illness and most have a co-occurring diagnosis of substance abuse. These are individuals who do not and will not come into the clinic setting for traditional treatment. The ACT Team takes the treatment services to them in their homes, shelters, sidewalks, etc. ACT is available 24 hours/7 days a week to individuals through an on-call rotation among ACT team members. Services will be provided after hours and on weekends. _
Qualifications and Requirements:
Licensed or Associate-Licensed (Social Worker, Marriage and Family Therapist, or Professional Counselor)
Previous experience working with individuals with a serious persistent mental illness and/or co-occurring substance abuse within a multidisciplinary team.
Previous case management experience, including experience with documentation, managing a caseload, conducting assessments, and providing brief interventions.
