    RN Corporate Director of Case Management
    Nashville, Tennessee

    The RN Corporate Director of Case Management identifies improvement opportunities and facilitate process standardization to promote improved outcomes, maximize Resource Utilization, and improve organizational efficiencies. The Corporate Director of Case Management works as a cross functional team member with various groups inside and outside the Case Management Department to develop and implement a comprehensive program to achieve desired strategic objectives. The Corporate Director of Case Management is primarily responsible for the assessment, planning, coordination, development, and direction of the Case Management/Transitional Care, Post-Acute Services support, Social Work programs, and Case Management related Regulatory Standards and other Case Management initiatives, including the areas of automation and information systems.

    Qualifications:

    • Registered Nurse required
    • Bachelor of Nursing required
    • Master’s degree preferred
    • Director of Case Management experience in an Acute Hospital setting required
    • Performance Improvement experience preferred

    Responsibilities:

    • Develops and implements evidence-based toolkits in support of Case Management initiatives
    • Conducts needs analyses, identifies gaps, and develops program components to mitigate identified gaps
    • Establishes, implements, and evaluates the strategic plan for clinical aspects of Case Management
    • Creates data priorities and report formats that display critical outcome, process and operational factors
    • Ensures all Case Management and Discharge Planning policies and procedures are consistent with governmental and regulatory agencies
    • Directs and develops Discharge Planning tools needed to support Case Management initiatives
    • Responsible for integration of Clinical Documentation and electronic health record technology into the program workflow
    • Ensures technology supports the Case Management Facility Departments
    • Provides ongoing evaluation and assessment of facility practices and industry changes to ensure enterprise remains current in Case Management processes
    • Supports programs including automation and information systems
    • Provides feedback to Case Management Leadership and other clinical areas
    • Provides input for resources needed to appropriately and effectively deliver Case Management strategies
    • Develops, establishes and maintains effective relationships with related departments as indicated to promote and ensure success of Case Management goals
    • Designs, develops, implements, evaluates, and revises the Case Management programs to ensure optimum patient outcomes
    • Develops creative suggestions to program development obstacles
    • Collaborates with other members of the Corporate Case
    • Management team to develop Case Management policies and procedures to support systems, standards of practice, regulatory standards, and evidence-based best practices
    • Develops Care Coordination education and training in conjunction with the Director of Case Management Education, regarding best practices, processes, and standards related to regulatory compliance issues related to Case Management
    • Responsibilities include Case Management/Transitional Care, Patient Choice and the Referral Process, Avoidable Days, ED Case Management, alignment with Post-Acute Services, Social Work programs and other Clinical Case Management
