RN Director of Case Management
Sonoma County, California

The RN Director of Case Management is responsible for Case Management Department operations.The Director of Case Management focuses on the achievement of satisfactory patient outcomes. The RN Director of Case Management is responsible for coordinating with appropriate staff to ensure a seamless transition of patients across the Continuum of Care. The RN Director of Case Management oversees the Utilization Management functions, policies and procedures as well as federal, state and local regulations.

Qualifications:

• Bachelor's Degree required
• Registered Nurse required
• Director of Case Management experience in an Acute Hospital Setting required
• Utilization Management and Case Management principles knowledge
• Case Management Certification preferred

Responsibilities:

• Directs development and implementation of quality and utilization standards across the hospital
• Ensures compliance with administrative, legal and regulatory requirements
• Maintains oversight of the concurrent and retrospective inpatient reviews and appeal process
• Utilizes research data to implement clinical changes and the delivery of patient care
• Evaluates processes and makes recommendations to improve Resource Management systems and patient care outcomes
• Develops processes to screen, interview, hire, train, and maintain the competency of all department staff
• Develops and manages systems and relationships with outside vendors
• Develops budget and resource allocations for areas of responsibility
• Manages the financial performance and identifies and implements appropriate strategies to reduce costs and improve quality of care and services
