RN Director of Case Management

The RN Director of Case Management oversees the overall operations of Care Management functions. The RN Director of Case Management is responsible for developing systems and processes for Case Management and Social Services and managing department activities related to clinical quality improvement. The RN Director of Case Management provides support and leadership to collaborative Discharge Planning and Utilization Review processes.

Qualifications:

• Registered Nurse
• Bachelor Degree in Nursing
• Master Degree
• Certified Case Manager (CCM)
• EPIC Knowledge
• Director of Case Management experience in a large acute care hospital
• For-profit acute Case Management leadership

Responsibilities:

• Coordinate duties with appropriate personnel to meet operational program needs, ensures compliance with state and federal health plan requirements, Medicare guidelines and standards; develops and implements policy and procedures, updates and integrates current clinical practice guidelines
• Drives results based on key performance indicators and problem solve
• Strong interpersonal skills with Physician Engagement and accountability
• Oversees the overall operations of Care Management functions through oversight of the hospital
• Supervisory principles and practices, assessment, planning, implementation and coordination of patient care
• Work with patients and staff in a variety of conditions
