Job Description:

We are seeking a detail-oriented Healthcare Referral Coordinator to manage patient referral processes between healthcare providers, specialists, and insurance companies. The ideal candidate will coordinate appointments, ensure timely follow-ups, and facilitate seamless communication between all parties involved in patient care.

Responsibilities:

Process patient referrals and ensure they are routed to the appropriate specialist or facility.

Verify patient information and insurance coverage to facilitate smooth referral handling.

Maintain accurate and up-to-date patient records within the healthcare system.

Coordinate and schedule appointments for patients with specialists.

Collaborate with provider(s) to ensure that referrals are processed in a timely manner.

Maintain strict HIPAA compliance

Assist with ensuring follow up documented in medical records ie “close the referral loop”

Update preventative health care information in patient’s medical record ie colon cancer, prostate cancer, breast cancer, cervical cancer screenings if relevant to the particular referral

Qualifications:

LPN or equivalent; associate degree or higher in healthcare administration a plus.

Experience in a medical or clinical setting, preferably with referral management.

Strong computer, organizational and multitasking skills.

Excellent communication skills and attention to detail.

$20.00 per hour, 15 hours per week to start

3 month period of mutual assessment

Continuing education encouraged
