Key Responsibilities

Direct Client Services

Provide community based support services

Deliver skills training, case management, and service coordination

Teach essential life skills, social skills, and coping mechanisms to help clients manage emotional and behavioral challenges

Implement crisis intervention strategies to reduce hospitalizations and provide early support

Coordinate care by connecting clients with medical, educational, and social service resources

Progress notes

Monitor daily operations to ensure smooth workflow and timely service delivery

Report to leadership on program performance, compliance issues, and corrective actions

Participate in audits, reviews, and surveys
