Location: On-site (Pediatric Primary Care Practice) Employment: Grant-funded (5-year term), Full-time or Part-time Reports To: Practice Administrator / Medical Director (With training and guidance from the HealthySteps National team)

HealthySteps is a national, evidence-based program that supports children ages 0–3 and their caregivers within pediatric primary care.

Position Summary
The HealthySteps Specialist is part of the pediatric care team. You will support families during well-child visits and follow-up interactions.

Parenting support
Early identification of concerns
Connecting families to services
This role provides short-term, preventive, and consultative support. It does not include long-term therapy or ongoing psychotherapy.
What a Typical Day May Include
Supporting Families
Promote the HealthySteps program to families and providers
Conduct well-child visits (in-person or virtual) focused on development and relational health
Provide guidance on child behavior, development, and parenting concerns
Build strong, trusting relationships with caregivers
Screening & Early Identification
Conduct developmental, behavioral, and social-emotional screenings
Share results with pediatric providers
Identify early signs of delays, behavioral concerns, or family stress
Care Coordination
Coordinate care with the primary care team
Participate in supervision and team meetings
Document visits, screenings, and referrals in the EHR
Accompany families to care appointments when appropriate
Referrals & Follow-Up
Connect families to community resources (early intervention, behavioral health, social services)
Support families in completing referrals
Follow up to ensure services are accessed
Use closed-loop referral practices when possible
Parent & Community Engagement
Facilitate parent support groups
Manage community referrals and follow-ups
Maintain a family support line and respond promptly
Program Participation
Track caseload and service levels
Participate in HealthySteps training and coaching
Follow HealthySteps guidelines and evidence-based practices
Document services to support program reporting and grant requirements
Practice Approach

Required Qualifications
Master’s degree in Social Work, Psychology, or Counseling
Licensed in New York State (LMSW, LCSW, LMHC, LPC, or equivalent)
Experience working with children ages 0–3 and caregivers
Strong communication and engagement skills
Ability to work as part of a clinical team
Comfort discussing sensitive topics with families
Bilingual Proficiency in English and Spanish is required
Preferred Qualifications
Experience using developmental screening tools
Familiarity with pediatric primary care or integrated behavioral health
Experience in grant-funded or evidence-based programs
