    This service coordination position is to monitor and advocate for consumer’s needs in the HCS waiver, General Revenue, and Texas Home Living program. The individual in this position should be able to effectively interact with consumers/families, service providers, as well as community resources. This position requires independent and objective judgment. The person in this position must work independently within specific guidelines; must be able to effectively work with the provider to develop the Person Directed Plan as well as IPC and IDRC. The person must be able to communicate effectively verbally and through written documentation. Experience in a social service setting is preferred.

    EXAMPLES OF WORK PERFORMED:

    1. Conduct interviews with consumers and families to gather information on likes, dislikes, goals, health and safety concerns, and needs.

    2. Coordinate the Service Planning Team including the individual, family, Service Coordinator, and others as requested by the individual/LAR.

    3. Identify community resources available to the individual that are non-waiver funded.

    4. Provide monitoring of consumers outcomes to ensure adequacy and quality of services being provided.

    5. Complete initial IPC and participate in the renewal and revision of IPC’s as required by HCS rule.

    6. Completing documentation related to service provision, including Person Directed Plans, Individual Plans of Care, ICSR’s, IDRC’s and CARE data entry as required.

    SALARY: $35,000.00 FOR BACHELOR’S LEVEL / $37,450.00 FOR MASTER’S LEVEL

    QUALIFIED AND VERIFIED YEARS OF EXPERIENCE MAY RESULT IN AN INCREASED STARTING RATE OF PAY.

    The Heart of Texas Behavioral Health Network (formerly Heart of Texas Region MHMR Center) strives to deliver accessible, caring and responsive support services to individuals and families coping with mental illness, intellectual and developmental disabilities, developmental delays, and emotional conflict. The Heart of Texas Region MHMR Center was established by local taxing authorities in 1968 and is currently sponsored by McLennan, Limestone, Hill, and Bosque Counties. We serve a six county area including McLennan, Limestone, Hill, Bosque, Freestone and Falls Counties. We provide services under contracts with federal, state, and other entities to individuals with a mental illness and/or a developmental disability identified as priority population. We also serve developmentally delayed children. The Center was renamed Heart of Texas Behavioral Health Network (doing business as the Heart of Texas Region MHMR Center) in December 2021 in a rebranding action approved by the Center's Board of Trustees. The Center also earned Texas Certified Community Behavioral Health Clinic (CCBHC) status in December, 2021. The Center employs around 450 full and part-time staff and is governed by a nine member Board of Trustees appointed by county commissioners.
