Home Health Care Coordinator

We are currently seeking a skilled and compassionate Home Health Care Coordinator to join our team. As a Home Health Care Coordinator, you will play a vital role in providing exceptional care to our clients and ensuring the smooth operation of our home health care services.

Responsibilities

· Conduct initial assessments of clients' health conditions and develop personalized care plans.

· Coordinate internal and external staffing needs.

· Assign clients to the appropriate clinicians based on their needs and availability.

· Utilize cloud-based scheduling software to manage staff schedules and appointments.

· Maintain an up-to-date list of on-call and backup staff and use it to secure last-minute coverage when necessary.

· Communicate with caregivers to inform them of any updates or changes to their schedule.

· Communicate with managers and clients to provide timely and accurate information regarding schedule or staffing changes.

· Assist in the hiring, training, and management of new staff members.

· Manage patient files and other documents related to patient care.

· Ensure compliance with all legal, regulatory, and accreditation standards.

Qualifications

· Any kind of nursing experience or assessment

· Minimum of one year of clinical experience, preferably in a home health or similar setting

· Previous experience as a Home Care Scheduler or in a similar position is preferred.

· Strong clinical assessment and critical thinking skills

· Excellent communication and interpersonal skills to effectively work with clients, families, and healthcare team members.

· Ability to prioritize and manage multiple tasks efficiently.

· Proficient computer skills for documentation and electronic medical records

· Highly organized with attention to detail

· Comfortable with Microsoft Office and other computer programs

· Ability to multitask and prioritize projects.

· Excellent customer service and interpersonal skills
