The Hepatitis C Linkage Specialist will play a key role in the Relink initiative, a national program aimed at connecting individuals diagnosed with hepatitis C to curative treatment. This position works directly with local health departments across 15 districts and partners with the Virginia Hepatitis C Education and Patient Connection (VA HEPC) team to ensure individuals who have tested positive for hepatitis C receive navigation, education, and support to access treatment.

Requirements:

• • Bachelor’s degree in public health, Health Sciences, Social Work, Nursing, Psychology, or related field OR equivalent experience in case management, patient navigation, community health work, or public health outreach.
  • • Strong communication skills, including the ability to engage individuals experiencing stigma, substance use, or complex social needs.
  • • Ability to maintain confidentiality and handle sensitive health information.
  • • Proficiency with computers, data entry, and electronic systems (e.g., Excel, SharePoint, EMR, surveillance systems).
  • • Valid driver’s license and ability to travel across local districts as needed.
