What You’ll Need to Apply:

To help get you started as soon as possible, please have the following documents ready:

Valid HHA or PCA Certification

Valid ID (Driver’s License, Passport, or State ID)

Social Security Card

Employment Authorization Card (if required by your Social Security card)

Pre-Employment Physical Form – must include MMR, TB test, and Drug Screen

(We can provide our physical form if needed)

COVID-19 Vaccine Card (if you received it)

Flu Vaccine (if you received it)

Hep B Vaccine (if you received it)
