Position Summary
The Clinical Team Lead (LPN) provides clinical oversight, staff supervision, and operational support across residential homes serving individuals with intellectual and developmental disabilities (IDD). This role ensures high-quality, compliant care delivery while supporting clients’ medical and daily living needs. The ideal candidate is a Licensed Practical Nurse with leadership experience in residential or community-based IDD settings and a flexible, hands-on approach.
Key Responsibilities
Clinical Oversight
Conduct initial nursing assessments and ongoing reassessments for clients
Review, monitor, and ensure accuracy of Medication Administration Records (MARs)
Manage medication reorders and coordinate with pharmacies
Monitor clients for changes in condition and report concerns appropriately
Accompany residents to medical appointments and communicate follow-up care plans
Provide clinical guidance to direct care staff
Staff Supervision & Training
Supervise field staff across residential homes
Train, onboard, and mentor new hires
Conduct competency evaluations and skills validations
Provide coaching, corrective action, and performance feedback
Serve as a clinical resource for staff questions and concerns
Operations & Compliance
Perform routine site visits to ensure compliance with state regulations and agency policies
Ensure homes are properly stocked with medical and household supplies
Monitor documentation, infection control, and safety practices
Assist with incident reports and quality assurance initiatives
Maintain regulatory readiness for audits and surveys
Staffing Support
Provide coverage for call-offs and staffing shortages when needed
Assist with schedule coordination to maintain continuity of care
Qualifications
Required
Active LPN license in good standing
Minimum 2 years of nursing experience with IDD population and/or residential care settings
Strong knowledge of medication administration and documentation
Proven supervisory or leadership experience
Reliable transportation and ability to travel between multiple locations
Flexible availability, including evenings and occasional weekends
Preferred
Experience in group homes, waiver programs, or long-term residential settings
Knowledge of Medicaid waiver services
Strong organizational, leadership, and communication skills
Ability to manage multiple priorities independently
Physical & Scheduling Requirements
Ability to lift up to 50 lbs as needed
Standing, walking, bending, and driving between locations
On-call availability as required
Must be adaptable to fluctuating schedules and staffing needs
