    Job Summary:

    Holy Angels Residential Facility is in search of a DSW/DSP Direct Service Professional to provide care and assist with daily living skills for individuals with intellectual and developmental disabilities.

    Specific Skills and Responsibilities

    · Supervise, assist, and train resident in all activities of daily living, including but not limited to, personal hygiene skills, clothing care skills, social skills, leisure skills, care of personal space, dining skills, toileting, and appropriate behavior skills

    · Provide continuous active treatment for residents

    · Accurately complete all assigned documentation including but not limited to, formal and service objectives, behavior incident reports, accident/incident documentation, medical related documentation

    · All documentation put in computer system

    · Report any issues or sign/symptoms of resident illness to nurse and supervisor

    · Provide additional services which may be assigned as part of the job such as laundry, dining room duty, or any other reasonable requests made by supervisor deemed necessary for resident care or as part of the job

    · Requires daily computer use

    Qualifications:

    · Must be able to take the CMA class as required for all DSP’s (class may be outside your shift)

    · Must maintain a current and valid driver’s license to drive vehicles

    Education:

    · GED or High School Diploma Required

    Work Environment:

    · Must be able to work with individuals with developmental disabilities

    Physical Demands:

    · Walk, stand, sit, push, pull, stoop, kneel, crouch, reach, use hands and lift up to 50lbs, 2/3’s of the time
